Request for Facilitated Meeting Form

Tribal meetings must be scheduled for a date within 30 calendar days but no fewer than 15 calendar days after this date, unless the applicant and the requestor agree to an earlier date
Requests made at least 15 calendar days prior to the scheduled hearing must take place before the hearing, or the case will be deferred.
Upload list from Office of Native American Affairs (Pre-submittal Tribal Meeting only). File should be less than 4 MB in size.
Upload list from Office of Neighborhood Coordination (Post-submittal Facilitated Meeting only). File should be less than 4 MB in size.
