Process for Reservation

Please review all rental information for the reservation process, cost, locations, facilities, stipulations, and requirements.

Please be advised: After-hours rental rates differ from standard facility use rates during regular operating hours.

1. Register for or Update Community Center Pass

Before continuing to the form below, please be sure you have an updated Adult Community Center Pass. Passes are free and must be created or updated prior to submitting the Facility Usage/Rental Interest Form. Please create your account and update your pass before you fill out the interest form. Record the account number listed next to your last name for the online Facility Usage/Rental Interest Form.

2. Fill out Online Request Below

Fields with a red dot are mandatory. Each upload must be 4 MB or smaller in size and the total size of all files you uploaded must be smaller than 8 MB.

I understand that this is an interest form and I will be contacted by the rental coordinator or community center staff regarding this request.

Please confirm all required fields are populated before submitting your form. Fields with a red dot are mandatory. Your form is not submitted until you click the Submit button and see the confirmation page which includes a recap of your completed form fields. If you do not see this page please double check that the total size of all files you uploaded is smaller than 8 MB. Uploading files that are larger than 8 MB total will result in a failed submission.
