We assist employees and supervisors in understanding and navigating FMLA, which provides eligible employees with up to 12 weeks of job-protected leave for certain family and medical reasons.

On January 14th, 2026, the City will be transitioning to a new electronic FMLA process conveniently available in Employee Self-Service. This updated process will streamline how employees apply for FMLA and submit required documentation, helping to improve efficiency, accuracy, and communication throughout the entire process.

Submit an FMLA eForm: Use this option to submit a new request for FMLA eligibility and receive the appropriate Health Certification.
Evaluate an FMLA eForm: Use this option to upload and submit completed Health Certifications and other supporting documentation for review.
Update an FMLA eForm: Use this option to update/edit eForms that are currently in Saved, Recycle (Push Back) or Pending status.
