Service Member, Veteran, and Family (SMVF) Service Provider Needs Questionnaire
The City of Albuquerque Veterans and Military Affairs Advisory Board (VMAAB) is collecting information about factors influencing the health and wellbeing of service members, veterans, and their families (SMVF). The goal is to identify emerging needs and service gaps that impact this community.
Purpose

Why We Are Gathering This Information
We are compiling data about SMVF mental health, physical health, housing stability, and related needs to identify ways to strengthen local partnerships and improve access to care and support services across Albuquerque.
This questionnaire should take approximately 5–10 minutes to complete.
The results of this survey will be posted online once they have been collected after a 60 day period and studied. The link will be shared publicly and sent out once it is available.
All responses will remain anonymous; no identifying or HIPAA-protected information will be collected.
