Police Complaint Form

A complaint must be submitted within 90 days of the incident.

The online form below initiates a complaint with the Independent Review Office. The Police Union contract with the City of Albuquerque requires complaints to be signed. Therefore, you will be required to sign the complaint at a later date, or, you may fax your signature to (505) 924-3775.

Please do not use this form if you need to make a police report. A police report can be made at any police station.

If you do not wish to file your complaint on-line, you may print the form and mail or fax it to us.
