Does the patient have housing options (family/friends, supportive housing, motel) or is in a residential care facility)?

If Hospital provider receives COVID19 test result for discharged patient experiencing homelessness:

Contact WEHC Medical Team at 505-738-3382 from 8 a.m. – 8 p.m. They will coordinate follow up for people staying at the Westside Emergency Housing Center, motels or sleeping rough.

Patients with tentative/confirmed COVID-19 & complicating health issues

Patients who have conditions that require ongoing nursing care, controlled substances or if the patient is likely to experience substance withdrawal in the next 24 hours, the hospital needs to follow its internal protocol. The Westside Emergency Housing Center (WEHC) does not have medical staff to provide complex care or substance use care.
