All notices need to have the proper information in order to be set up and investigated in a timely manner; you have several options for filing your claim:

The best place to start your claim is with your insurance company. They have the expertise to manage the process and mitigate your damages in a timely manner. But you can also file directly with the City using the claim submission form.
As stated in the New Mexico Tort Claims Act, in most cases you have ninety (90) days from the date of the incident to file your claim with the City.
Be aware that if you miss this deadline, your claim will be denied.
